Application Process for the Program in Occupational Therapy

Formal application is made to the Program in Occupational Therapy using the on-line application between November 15, 2008 and August 15, 2009 (see link below). 

An official transcript from each college and university attended should be mailed from the schools' Office of the Registrar directly to:

A $55.00 application processing fee should be mailed to the above address.  Make your check or money order payable to:  University of Minnesota

If you have any supplemental materials (eg. prerequisite course reviews) please mail them to the above address.

Applications will be reviewed only after receipt of all transcripts and the application processing fee.

Our online application for Fall 2009 will be available after November 15, 2008.  There will be some changes made to the 2009 application but you may review our old application by clicking on the link below.
